Anyone looking into ADHD will quickly come across a wide variety of different testing methods. From short online questionnaires to in-depth interviews, there seem to be countless options. But not every test serves the same purpose. While some methods merely provide an initial assessment, others are an integral part of a professional diagnosis.
1. Structured Interviews—the Gold Standard
Structured interviews are considered the most important tool in ADHD diagnosis. The DIVA-5 (Diagnostic Interview for ADHD in Adults) is used particularly frequently. This is a scientifically developed interview based on the diagnostic criteria of the DSM-5. Together with a specialist, typical ADHD symptoms are systematically discussed and compared with the individual’s personal history. This allows for an assessment of whether the symptoms actually fit the profile of ADHD or whether other causes are more likely.
2. Self- and Peer-Assessment Questionnaires
In addition to structured interviews, standardized questionnaires are frequently used. They help systematically identify typical symptoms and supplement the individual’s self-perception with the assessments of family members or other significant others. Among the best-known tools are:
- ASRS (Adult ADHD Self-Report Scale): A short questionnaire co-developed by the World Health Organization (WHO) that is frequently used worldwide as the initial screening tool for adults.
- WURS-k (Wender Utah Rating Scale): This questionnaire looks back at childhood and helps assess whether typical ADHD signs were already present at a young age.
- CAARS (Conners’ Adult ADHD Rating Scales): A more comprehensive assessment tool that covers various areas of daily life and can be used for both self-report and third-party assessment.
These assessments provide important insights but do not replace a professional diagnosis.
3. Attention and Concentration Tests
Neuropsychological testing procedures can be used as a supplement. These include, for example, the Continuous Performance Test (CPT), the Stroop Test, or the Trail Making Test (TMT). Under standardized conditions, they assess, among other things, attention, the ability to concentrate, reaction speed, and impulse control. However, since these tests provide only a snapshot in time, they are always evaluated in conjunction with other test results.
How reliable are the various testing methods?
Not every ADHD test provides equally reliable results. While some tests merely offer an initial orientation, others are considered a key component of professional diagnosis. The decisive factor is always the context in which a test is used and how its results are interpreted.
1. Low reliability: Online self-tests
Online self-tests are often the first point of contact with the topic of ADHD. They can help users better understand typical symptoms and provide clues about a possible attention-deficit/hyperactivity disorder. However, their reliability is limited. The results are based solely on self-assessment and do not take into account either the individual’s medical history or other possible causes of the symptoms. Therefore, online tests are suitable only as a preliminary guide.
2. Moderate Reliability: Questionnaires and Attention Tests
Standardized questionnaires such as the ASRS, WURS-k, or CAARS, as well as attention tests such as the CPT, Stroop Test, or Trail Making Test (TMT), provide additional information about typical ADHD symptoms or the ability to concentrate. On their own, however, they are not sufficient to definitively diagnose ADHD. Some individuals with ADHD may perform normally in a test setting, while other conditions can produce similar results. For this reason, these methods are always evaluated in conjunction with other test results.
3. High Diagnostic Value: The Combination of Different Methods
The highest diagnostic value does not come from a single test, but rather from the interaction of various diagnostic components. These include, among other things, a structured interview such as the DIVA-5, the individual’s medical history, information from childhood—such as old school report cards—as well as standardized questionnaires and, if necessary, additional testing procedures. Only after all information has been compiled and other possible causes have been ruled out can ADHD be reliably diagnosed.
What happens after an ADHD test shows an abnormal result?
An abnormal result on an online self-test or questionnaire does not automatically mean that ADHD is present. It merely indicates that typical symptoms may be present and that a more detailed evaluation is warranted. The next step, therefore, is not to self-diagnose, but to review the results with a specialist.

The diagnostic process begins with a detailed discussion. This conversation focuses not only on your current symptoms but also on how they manifest in your daily life and how long they’ve been present. Specialists want to understand the challenges you face at work, in school, or in your personal life, and to what extent they impact your daily routine. At the same time, they consider other possible causes that could lead to similar symptoms.
2. Further tests help complete the picture
Depending on the situation, additional diagnostic procedures may be used afterward. These include structured interviews such as the DIVA-5, standardized questionnaires, or neuropsychological tests. Information from childhood—such as old school report cards or conversations with family members—can also provide important insights. The decision about which tests are necessary is always made on an individual basis. Not everyone goes through exactly the same diagnostic steps.
3. When Is ADHD Actually Diagnosed?
An ADHD diagnosis is only made once all test results have been evaluated together and the official diagnostic criteria have been met. In doing so, specialists consider not only individual test results but the overall picture comprising symptoms, life history, and other findings. It is precisely this comprehensive approach that ensures the diagnosis is as reliable as possible and that appropriate treatment can be planned.
